I Consent to a Pre-Employment test to detect the illegal use of controlled substance, alcohol, or prescription medication without a prescription. Medication without a prescription. Elite HR reserves the right to screen, test, or otherwise evaluate for alcohol and drug use those whose employment might present serious safety risks of alcohol or drug abuse were abuse present. If a screen results in positive the applicant has the option to undergo clinical test or decline going further in application process. If the clinical results are "positive" the applicant will not be eligible for hire and is liable for the cost of the clinical test. IF the clinical results are "negative", the application may resume, and will Elite HR accrue the cost of the clinical test.

Elite HR reserves the right to test, screen, or otherwise: medically evaluate all alcohol and drug abuse. Such instances may include, but not limited to probable cause, post-injury, random or systemic basis at any time Elite HR deems necessary. When an associate is screened, testing, or evaluations will render the same results as a "positive" the associate will be placed on immediate suspension from work and removed frm the work site until confirmation of clinical tests results are provides from medical facility.

A refusal to submit to screening, testing or evaluations will render the same results as a "positive" test result confirmation, namely, ineligibility for hire, and if currently as associate, immediate termination from assignment

Tampering or attempting to tamper with a specimen sample will render the same results as a "positive" test result confirmation, namely, ineligibility for hire, and if currently an associate, immediate termination from assignment

I understand and agree that Elite HR may release the results of my pre-employment and/or post accident drug screens to the state unemployment department if a claim for unemployment insurance is filed by me or on my behalf

I agree to hold all parties harmless and not to sue in connection with any aspect of drug screen testing or its effect on my employment status. I understand that if I have any questions about the meaning of the provisions in this authorization and consent or drug screen, they will be answered on request

AT-WILL EMPLOYMENT
Employment at your worksite employer and Elite HR is "AT-WILL." The employment relationship may be terminated for any reason with or without cause or cause or notice at any time by you or either. No oral statement shall limit the right to terminate employment at-will.
